Immunizations
Minnesota law requires that every child attending school be immunized against diphtheria, tetanus, pertussis, polio, measles, mumps, rubella, hepatitis B and varicella (hepatitis A and meningococcal beginning September 2014). The chart "Are Your Kids Ready" outlines the immunization requirements. A student must have all immunizations up-to-date, with a record on file in the health office, or the appropriate form signed, prior to starting school.
No student shall be required to receive an immunization which is contrary to a medical reason or conscientiously held belief. If you are conscientiously opposed to immunizations your notarized signature is required on file in our office. Please click here for the appropriate copy of the form.
